Cost of ABA therapy in Virginia
The honest version: with Virginia Medicaid, most families pay nothing out of pocket. With private insurance, you pay your plan’s normal cost-sharing. Here are the real numbers and where they come from.
The numbers in Virginia
WITH MEDICAID
$0
out of pocket for most families
Virginia Medicaid covers ABA for eligible kids under 21 and pays providers directly. Your job is the paperwork — and we do that part with you.
WHAT THE STATE PAYS PROVIDERS
$60–$187/hr
for one-on-one ABA (97153-type services)
Published rates vary by clinician credential (technician vs. BCBA) and setting, which is why this is a range — never one magic number. Source: Virginia’s published Medicaid fee information. This is the state’s bill, not yours.
WITH PRIVATE INSURANCE
Your plan’s normal cost-sharing
Virginia law requires state-regulated plans to cover autism care (on the books since 2011). You pay deductibles and copays like any other medical care — and we tell you the exact number for your plan before you start.
Rates and rules change on the state’s schedule, not ours — we keep this page current, but your plan’s written verification is the number that counts. That’s why the coverage check comes first, free.
Virginia cost questions
For most Medicaid families, nothing out of pocket. Virginia Medicaid covers ABA for eligible children as a medical benefit, and the state pays the provider directly. Behind the scenes, Virginia Medicaid reimburses providers roughly $60 to $187 per hour for one-on-one ABA, depending on the clinician's credential level and setting — but that is the state's bill, not yours.
Your plan's normal cost-sharing — deductible, copays or coinsurance, up to your out-of-pocket maximum. The reason coverage exists at all: Enacted 2011 (HB 2467, effective 1/1/2012) initially for ages 2-6; age cap raised to 10 in 2015 and eliminated in 2019 (HB 2577); ABA subject to a $35,000 maximum annual benefit; extended to individual/small-group plans in 2020
Those figures are full-price provider billing across many weekly hours — the amount insurers negotiate against, not what covered families pay. Once Medicaid or a covered private plan is in place, your cost is your plan's cost-sharing, which is a completely different number.

So autism insurance coverage in Virginia is really two questions: what the state mandates, and what kind of plan you actually hold. Your plan documents or HR will tell you which one you have.
Yes. Virginia Medicaid ABA therapy coverage is not optional for the state: it covers medically necessary ABA for eligible children — that obligation comes from the federal EPSDT requirement, so it holds in every state. What differs here is the route in: ABA covered for members under 21 through the EPSDT benefit as DMAS Behavioral Therapy services under Cardinal Care; delivered via Medicaid managed-care plans (Aetna, Anthem HealthKeepers Plus, Sentara, UnitedHealthcare) or fee-for-service Pediatric ABA therapy in Virginia still needs a current diagnostic evaluation on file and, in most cases, prior authorization before sessions start.
Paying cash is the last option to consider, not the first. Before that, check three things: whether your child qualifies for Virginia Medicaid on their own disability regardless of household income, which many families assume they don't; whether a waiver applies — in Virginia: Three DD waivers administered with DBHDS: Community Living (CL), Family and Individual Supports (FIS), Building Independence (BI); apply via local Community Services Board; and whether your employer plan excludes ABA outright or merely requires authorization you haven't filed yet. Those three account for most of the families who believe they have no coverage.
