Cost of ABA therapy in Pennsylvania
The honest version: with Pennsylvania Medicaid, most families pay nothing out of pocket. With private insurance, you pay your plan’s normal cost-sharing. Here are the real numbers and where they come from.
The numbers in Pennsylvania
WITH MEDICAID
$0
out of pocket for most families
Pennsylvania Medicaid covers ABA for eligible kids under 21 and pays providers directly. Your job is the paperwork — and we do that part with you.
WHAT THE STATE PAYS PROVIDERS
$51/hr
for one-on-one ABA (97153-type services)
Published rates vary by clinician credential (technician vs. BCBA) and setting, which is why this is a range — never one magic number. Source: Pennsylvania’s published Medicaid fee information. This is the state’s bill, not yours.
WITH PRIVATE INSURANCE
Your plan’s normal cost-sharing
Pennsylvania law requires state-regulated plans to cover autism care (on the books since 2008). You pay deductibles and copays like any other medical care — and we tell you the exact number for your plan before you start.
Rates and rules change on the state’s schedule, not ours — we keep this page current, but your plan’s written verification is the number that counts. That’s why the coverage check comes first, free.
Pennsylvania cost questions
For most Medicaid families, nothing out of pocket. Pennsylvania Medicaid covers ABA for eligible children as a medical benefit, and the state pays the provider directly. Behind the scenes, Pennsylvania Medicaid reimburses providers about $51 per hour for one-on-one ABA, depending on the clinician's credential level and setting — but that is the state's bill, not yours.
Your plan's normal cost-sharing — deductible, copays or coinsurance, up to your out-of-pocket maximum. The reason coverage exists at all: Act 62 enacted 2008 - first-in-nation autism mandate; $36,000/yr benefit maximum for individuals under 21; a Nov 2023 PA Insurance Department notice requires autism to be covered as a mental health condition under federal MHPAEA parity from Jan 1 2024, effectively superseding the dollar/age caps
Those figures are full-price provider billing across many weekly hours — the amount insurers negotiate against, not what covered families pay. Once Medicaid or a covered private plan is in place, your cost is your plan's cost-sharing, which is a completely different number.
Act 62 enacted 2008 - first-in-nation autism mandate; $36,000/yr benefit maximum for individuals under 21; a Nov 2023 PA Insurance Department notice requires autism to be covered as a mental health condition under federal MHPAEA parity from Jan 1 2024, effectively superseding the dollar/age caps A mandate only reaches state-regulated plans, though — a self-funded employer plan is governed by federal law and can set its own terms regardless of what Pennsylvania requires. So autism insurance coverage in Pennsylvania is really two questions: what the state mandates, and what kind of plan you actually hold. Your plan documents or HR will tell you which one you have.
Yes. Pennsylvania Medicaid ABA therapy coverage is not optional for the state: it covers medically necessary ABA for eligible children — that obligation comes from the federal EPSDT requirement, so it holds in every state. What differs here is the route in: Covered for under-21s through Medical Assistance (Medicaid) EPSDT as Intensive Behavioral Health Services - ABA (IBHS-ABA, replaced BHRS in 2020); delivered through the HealthChoices behavioral health carve-out via county BH-MCOs (e.g. Magellan, Community Care Behavioral Health, PerformCare) Pediatric ABA therapy in Pennsylvania still needs a current diagnostic evaluation on file and, in most cases, prior authorization before sessions start.
Paying cash is the last option to consider, not the first. Before that, check three things: whether your child qualifies for Pennsylvania Medicaid on their own disability regardless of household income, which many families assume they don't; whether a waiver applies — in Pennsylvania: ODP waivers serving autism/ID: Consolidated, Community Living and Person/Family Directed Support (P/FDS) waivers; autism-specific adult programs: Adult Autism Waiver (AAW) and Adult Community Autism Program (ACAP); and whether your employer plan excludes ABA outright or merely requires authorization you haven't filed yet. Those three account for most of the families who believe they have no coverage.
Ask about waivers before assuming you'll pay cash. In Pennsylvania: ODP waivers serving autism/ID: Consolidated, Community Living and Person/Family Directed Support (P/FDS) waivers; autism-specific adult programs: Adult Autism Waiver (AAW) and Adult Community Autism Program (ACAP)
