Cost of ABA therapy in Missouri
The honest version: with Missouri Medicaid, most families pay nothing out of pocket. With private insurance, you pay your plan’s normal cost-sharing. Here are the real numbers and where they come from.
The numbers in Missouri
WITH MEDICAID
$0
out of pocket for most families
Missouri Medicaid covers ABA for eligible kids under 21 and pays providers directly. Your job is the paperwork — and we do that part with you.
WHAT THE STATE PAYS PROVIDERS
$65–$81/hr
for one-on-one ABA (97153-type services)
Published rates vary by clinician credential (technician vs. BCBA) and setting, which is why this is a range — never one magic number. Source: Missouri’s published Medicaid fee information. This is the state’s bill, not yours.
WITH PRIVATE INSURANCE
Your plan’s normal cost-sharing
Missouri law requires state-regulated plans to cover autism care (on the books since 2010). You pay deductibles and copays like any other medical care — and we tell you the exact number for your plan before you start.
Rates and rules change on the state’s schedule, not ours — we keep this page current, but your plan’s written verification is the number that counts. That’s why the coverage check comes first, free.
Missouri cost questions
For most Medicaid families, nothing out of pocket. Missouri Medicaid covers ABA for eligible children as a medical benefit, and the state pays the provider directly. Behind the scenes, Missouri Medicaid reimburses providers roughly $65 to $81 per hour for one-on-one ABA, depending on the clinician's credential level and setting — but that is the state's bill, not yours.
Your plan's normal cost-sharing — deductible, copays or coinsurance, up to your out-of-pocket maximum. The reason coverage exists at all: HB 1311 (2010, effective Jan 1, 2011): state-regulated plans cover ASD treatment including ABA to age 18; $40,000/yr ABA cap is inflation-adjusted annually ($57,311 for 2025)
Those figures are full-price provider billing across many weekly hours — the amount insurers negotiate against, not what covered families pay. Once Medicaid or a covered private plan is in place, your cost is your plan's cost-sharing, which is a completely different number.
HB 1311 (2010, effective Jan 1, 2011): state-regulated plans cover ASD treatment including ABA to age 18; $40,000/yr ABA cap is inflation-adjusted annually ($57,311 for 2025) A mandate only reaches state-regulated plans, though — a self-funded employer plan is governed by federal law and can set its own terms regardless of what Missouri requires. So autism insurance coverage in Missouri is really two questions: what the state mandates, and what kind of plan you actually hold. Your plan documents or HR will tell you which one you have.
Yes. Missouri Medicaid ABA therapy coverage is not optional for the state: it covers medically necessary ABA for eligible children — that obligation comes from the federal EPSDT requirement, so it holds in every state. What differs here is the route in: MO HealthNet state-plan ABA for under-21 (EPSDT/Healthy Children and Youth) with ASD diagnosis per 13 CSR 70-98.030; FFS or via managed care — Home State Health, Healthy Blue, UnitedHealthcare, plus Show Me Healthy Kids Pediatric ABA therapy in Missouri still needs a current diagnostic evaluation on file and, in most cases, prior authorization before sessions start.
Paying cash is the last option to consider, not the first. Before that, check three things: whether your child qualifies for Missouri Medicaid on their own disability regardless of household income, which many families assume they don't; whether a waiver applies — in Missouri: DMH Division of DD waivers: Comprehensive Waiver; Community Support Waiver; MO Children with DD Waiver (MOCDD, Sarah Jian Lopez Waiver); Partnership for Hope; and whether your employer plan excludes ABA outright or merely requires authorization you haven't filed yet. Those three account for most of the families who believe they have no coverage.
Ask about waivers before assuming you'll pay cash. In Missouri: DMH Division of DD waivers: Comprehensive Waiver; Community Support Waiver; MO Children with DD Waiver (MOCDD, Sarah Jian Lopez Waiver); Partnership for Hope
