Cost of ABA therapy in Maryland
The honest version: with Maryland Medicaid, most families pay nothing out of pocket. With private insurance, you pay your plan’s normal cost-sharing. Here are the real numbers and where they come from.
The numbers in Maryland
WITH MEDICAID
$0
out of pocket for most families
Maryland Medicaid covers ABA for eligible kids under 21 and pays providers directly. Your job is the paperwork — and we do that part with you.
WHAT THE STATE PAYS PROVIDERS
$77/hr
for one-on-one ABA (97153-type services)
Published rates vary by clinician credential (technician vs. BCBA) and setting, which is why this is a range — never one magic number. Source: Maryland’s published Medicaid fee information. This is the state’s bill, not yours.
WITH PRIVATE INSURANCE
Your plan’s normal cost-sharing
Maryland law requires state-regulated plans to cover autism care (on the books since 2000). You pay deductibles and copays like any other medical care — and we tell you the exact number for your plan before you start.
Rates and rules change on the state’s schedule, not ours — we keep this page current, but your plan’s written verification is the number that counts. That’s why the coverage check comes first, free.
Maryland cost questions
For most Medicaid families, nothing out of pocket. Maryland Medicaid covers ABA for eligible children as a medical benefit, and the state pays the provider directly. Behind the scenes, Maryland Medicaid reimburses providers about $77 per hour for one-on-one ABA, depending on the clinician's credential level and setting — but that is the state's bill, not yours.
Your plan's normal cost-sharing — deductible, copays or coinsurance, up to your out-of-pocket maximum. The reason coverage exists at all: Habilitative-services mandate (Ins. §15-835; original mandate 2000, first-in-nation habilitative benefit); March 2014 regulation explicitly required ABA for children with ASD under 19 in state-regulated plans
Those figures are full-price provider billing across many weekly hours — the amount insurers negotiate against, not what covered families pay. Once Medicaid or a covered private plan is in place, your cost is your plan's cost-sharing, which is a completely different number.
Habilitative-services mandate (Ins. §15-835; original mandate 2000, first-in-nation habilitative benefit); March 2014 regulation explicitly required ABA for children with ASD under 19 in state-regulated plans A mandate only reaches state-regulated plans, though — a self-funded employer plan is governed by federal law and can set its own terms regardless of what Maryland requires. So autism insurance coverage in Maryland is really two questions: what the state mandates, and what kind of plan you actually hold. Your plan documents or HR will tell you which one you have.
Yes. Maryland Medicaid ABA therapy coverage is not optional for the state: it covers medically necessary ABA for eligible children — that obligation comes from the federal EPSDT requirement, so it holds in every state. What differs here is the route in: Maryland Medicaid ABA benefit (added 2017 under EPSDT) for members under 21 with ASD; fee-for-service benefit administered statewide through the Behavioral Health ASO Carelon Behavioral Health (replaced Optum 1/1/2025), regardless of MCO Pediatric ABA therapy in Maryland still needs a current diagnostic evaluation on file and, in most cases, prior authorization before sessions start.
Paying cash is the last option to consider, not the first. Before that, check three things: whether your child qualifies for Maryland Medicaid on their own disability regardless of household income, which many families assume they don't; whether a waiver applies — in Maryland: Maryland Autism Waiver (MDH + MSDE-administered; registry 866-417-3480; respite, family training, env. mods — supplemental to, not required for, Medicaid ABA; long waitlist); and whether your employer plan excludes ABA outright or merely requires authorization you haven't filed yet. Those three account for most of the families who believe they have no coverage.
Ask about waivers before assuming you'll pay cash. In Maryland: Maryland Autism Waiver (MDH + MSDE-administered; registry 866-417-3480; respite, family training, env. mods — supplemental to, not required for, Medicaid ABA; long waitlist)
