Cost of ABA therapy in District of Columbia
The honest version: with District of Columbia Medicaid, most families pay nothing out of pocket. With private insurance, you pay your plan’s normal cost-sharing. Here are the real numbers and where they come from.
The numbers in District of Columbia
WITH MEDICAID
$0
out of pocket for most families
District of Columbia Medicaid covers ABA for eligible kids under 21 and pays providers directly. Your job is the paperwork — and we do that part with you.
WHAT THE STATE PAYS PROVIDERS
$110/hr
for one-on-one ABA (97153-type services)
Published rates vary by clinician credential (technician vs. BCBA) and setting, which is why this is a range — never one magic number. Source: District of Columbia’s published Medicaid fee information. This is the state’s bill, not yours.
WITH PRIVATE INSURANCE
Your plan’s normal cost-sharing
District of Columbia law requires state-regulated plans to cover autism care (on the books since 2006). You pay deductibles and copays like any other medical care — and we tell you the exact number for your plan before you start.
Rates and rules change on the state’s schedule, not ours — we keep this page current, but your plan’s written verification is the number that counts. That’s why the coverage check comes first, free.
District of Columbia cost questions
For most Medicaid families, nothing out of pocket. District of Columbia Medicaid covers ABA for eligible children as a medical benefit, and the state pays the provider directly. Behind the scenes, District of Columbia Medicaid reimburses providers about $110 per hour for one-on-one ABA, depending on the clinician's credential level and setting — but that is the state's bill, not yours.
Your plan's normal cost-sharing — deductible, copays or coinsurance, up to your out-of-pocket maximum. The reason coverage exists at all: Habilitative services mandate (D.C. Law 16-198, 2006) requires fully-insured plans to cover ABA/OT/PT/speech for autism; 2013-2014 amendments explicitly named ABA in the habilitative-services definition; no arbitrary session or dollar caps when medically necessary
Those figures are full-price provider billing across many weekly hours — the amount insurers negotiate against, not what covered families pay. Once Medicaid or a covered private plan is in place, your cost is your plan's cost-sharing, which is a completely different number.
Habilitative services mandate (D.C. Law 16-198, 2006) requires fully-insured plans to cover ABA/OT/PT/speech for autism; 2013-2014 amendments explicitly named ABA in the habilitative-services definition; no arbitrary session or dollar caps when medically necessary A mandate only reaches state-regulated plans, though — a self-funded employer plan is governed by federal law and can set its own terms regardless of what District of Columbia requires. So autism insurance coverage in District of Columbia is really two questions: what the state mandates, and what kind of plan you actually hold. Your plan documents or HR will tell you which one you have.
Yes. District of Columbia Medicaid ABA therapy coverage is not optional for the state: it covers medically necessary ABA for eligible children — that obligation comes from the federal EPSDT requirement, so it holds in every state. What differs here is the route in: ABA covered under EPSDT via State Plan Amendment 23-0009 (approved 8/25/2023, effective 10/1/2023); access runs largely through Medicaid managed-care plans - AmeriHealth Caritas DC, MedStar Family Choice, CareFirst CHPDC, and HSCSN for children with special needs Pediatric ABA therapy in District of Columbia still needs a current diagnostic evaluation on file and, in most cases, prior authorization before sessions start.
Paying cash is the last option to consider, not the first. Before that, check three things: whether your child qualifies for District of Columbia Medicaid on their own disability regardless of household income, which many families assume they don't; whether a waiver applies — in District of Columbia: DDS-administered HCBS waivers: IDD Waiver and Individual and Family Support (IFS) Waiver (adults 18+, IFS capped $75,000/yr; DDS waiver unit 202-730-1556); HSCSN plan for children on SSI; and whether your employer plan excludes ABA outright or merely requires authorization you haven't filed yet. Those three account for most of the families who believe they have no coverage.
Ask about waivers before assuming you'll pay cash. In District of Columbia: DDS-administered HCBS waivers: IDD Waiver and Individual and Family Support (IFS) Waiver (adults 18+, IFS capped $75,000/yr; DDS waiver unit 202-730-1556); HSCSN plan for children on SSI
